{"id":5257,"date":"2024-12-11T17:24:43","date_gmt":"2024-12-11T16:24:43","guid":{"rendered":"https:\/\/irccs.com\/patologie\/uterine-fibroids\/"},"modified":"2026-03-30T09:41:31","modified_gmt":"2026-03-30T07:41:31","slug":"uterine-fibroids","status":"publish","type":"pathology","link":"https:\/\/irccs.com\/en\/pathology\/uterine-fibroids\/","title":{"rendered":"Uterine fibroids"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\"><span  id=\"pathology\" class=\"h2_anchor\"><\/span>Pathology<\/h2>\n\n\n\n<p><strong>Uterine myomas<\/strong>, also called <strong>leiomyomas<\/strong> or <strong>fibroids<\/strong>, are <strong>benign tumors<\/strong> that develop in the smooth muscle of the uterus. They are characterized by hyperdevelopment of smooth muscle cells and connective tissue of the uterus.<\/p>\n\n\n\n<p>Their <strong>incidence<\/strong> varies between <strong>20-25% and 40%<\/strong> in women of childbearing age, with differences related to factors such as <strong>ethnicity<\/strong> and <strong>family history<\/strong>.<\/p>\n\n\n\n<p>The possibility of a <strong>benign uterine myoma<\/strong> mimicking a <strong>uterine sarcoma<\/strong> is <strong>extremely rare<\/strong>, estimated to be around <strong>0.03%<\/strong>. <strong>Uterine sarcoma<\/strong> is a rare malignancy that accounts for about <strong>7% of all uterine malignancies<\/strong>.<\/p>\n\n\n\n<p>In most cases, with the arrival of <strong>menopause<\/strong> and the consequent <strong>reduction in hormone levels<\/strong>, <strong>uterine myomas<\/strong> tend to <strong>shrink in size<\/strong>, resulting in <strong>alleviation or disappearance of symptoms<\/strong>.<\/p>\n\n\n\n<p>The Candiolo Institute is involved in the <strong>diagnosis and treatment<\/strong> of this disease, with a focus on two key aspects: understanding whether there is a risk that the disease may evolve into malignancy and recognizing atypical forms early, which require early intervention.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Types<\/h3>\n\n\n\n<p>Fibroids can have different localizations in the uterus:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>&#8220;<strong>Subserous<\/strong>&#8221; myoma, develops outside the uterus;<\/li>\n\n\n\n<li>&#8220;<strong>submucosal<\/strong>&#8221; myoma, develops within the uterine cavity;<\/li>\n\n\n\n<li>&#8220;<strong>intramural<\/strong>&#8221; myoma; develops within the uterine muscle (myometrium).<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"symptoms\" class=\"h2_anchor\"><\/span>Symptoms<\/h2>\n\n\n\n<p>In some cases uterine myomas are <strong>asymptomatic.<\/strong> In others, depending on their size, number and location, symptoms may manifest with:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Abdominal, sacral or lumbar pain<\/strong>;<\/li>\n\n\n\n<li><strong>abdominal bloating<\/strong>;<\/li>\n\n\n\n<li><strong>sense of pelvic weight<\/strong>;<\/li>\n\n\n\n<li><strong>constipation<\/strong>;<\/li>\n\n\n\n<li><strong>urination disorders.<\/strong><\/li>\n<\/ul>\n\n\n\n<p>It should also be pointed out that some uterine fibroids, some submucosal ones, can cause <strong>severe genital bleeding <\/strong>as well as <strong>infertility<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"risk-factors\" class=\"h2_anchor\"><\/span>Risk factors<\/h2>\n\n\n\n<p>Risk factors for <strong>uterine myomas<\/strong> include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Age:<\/strong> most common between the ages of 40 and 50;<\/li>\n\n\n\n<li><strong>Familiarity:<\/strong> increased risk if the mother had fibroids;<\/li>\n\n\n\n<li><strong>ethnicity:<\/strong> more frequent in African American women;<\/li>\n\n\n\n<li><strong>Obesity: <\/strong>high body mass index increases risk;<\/li>\n\n\n\n<li><strong>Early menarche:<\/strong> onset of menstruation before age 10;<\/li>\n\n\n\n<li><strong>nulliparity:<\/strong> absence of pregnancy;<\/li>\n\n\n\n<li><strong>Alcohol: <\/strong>excessive consumption may increase risk;<\/li>\n\n\n\n<li><strong>Diet: <\/strong>high consumption of red meat and high-fat dairy products;<\/li>\n\n\n\n<li><strong>Chronic stress:<\/strong> may contribute to the development of myomas.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"diagnosis-and-examination\" class=\"h2_anchor\"><\/span>Diagnosis and examination<\/h2>\n\n\n\n<p><strong>Uterine myomas<\/strong> are often discovered <strong>incidentally<\/strong> during a routine <a href=\"https:\/\/irccs.com\/en\/divisioni-cliniche\/gynecologic-oncology-and-hereditary-tumors\/\" type=\"medical-area\" id=\"5119\">gynecologic examination<\/a><a href=\"https:\/\/irccs.com\/aree-mediche\/ginecologia-oncologica\/\"> <\/a>.<\/p>\n\n\n\n<p>For an <strong>accurate diagnosis<\/strong> they are critical:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Detailed medical history<\/strong>, collecting the patient&#8217;s symptoms and needs;<\/li>\n\n\n\n<li><strong>Gynecological objective examination<\/strong>;<\/li>\n\n\n\n<li><strong>Pelvic ultrasonography<\/strong>, transvaginal and transabdominal, which allows <strong>localization of myomas<\/strong> and evaluation of their ultrasonographic characteristics.<\/li>\n<\/ul>\n\n\n\n<p>In some cases it may be useful to perform a<strong>diagnostic hysteroscopy<\/strong>, a minimally invasive, rapid examination that can be performed without anesthesia and allows <strong>direct observation of the uterine cavity<\/strong>.<\/p>\n\n\n\n<p>If clinical or ultrasonographic features are not typical and <strong>malignant degeneration<\/strong> is suspected, the specialist may request further investigation by <strong>Nuclear Magnetic Resonance Imaging (MRI<\/strong>).<\/p>\n\n\n\n<p>Myomas must be distinguished from other conditions, such as<strong>adenomyosis<\/strong> (<a href=\"https:\/\/irccs.com\/en\/patologie\/endometriosis\/\" type=\"pathology\" id=\"5248\">endometriosis of the uterus<\/a>) or other uterine masses, including <strong>sarcomas<\/strong>, which are otherwise very rare; the definite diagnosis of sarcomas is usually made by <strong>histologic analysis after surgery.<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"therapies\" class=\"h2_anchor\"><\/span>Therapies<\/h2>\n\n\n\n<p>In cases of <strong>small asymptomatic myomas<\/strong>, <strong>periodic clinical and instrumental monitoring<\/strong> to control their size and characteristics is sufficient.<\/p>\n\n\n\n<p>When fibroids become <strong>symptomatic<\/strong> or reach significant size, treatment options include:<\/p>\n\n\n\t<div id=\"block_69c677a778f95\" class=\"accordion-block acf-block\">\n\t\t<div class=\"my-4 lg:my-6\">\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Surgical therapy<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p>Surgical treatment is indicated in more complex cases or when medication is not sufficient.<\/p>\n<p>It consists of the<strong>removal of the myoma<\/strong> (myomectomy) and can be performed <strong>hysteroscopically<\/strong> or more frequently <strong>laparotomically<\/strong>, depending on location, size, number of fibroids, age of the patient, and her reproductive desire.<\/p>\n<p><!-- \/wp:paragraph --> <!-- wp:paragraph --><\/p>\n<p>In case the patient has exhausted her reproductive desire, prophylactic uterine and tubal removal surgery (hysterectomy and salpingectomy\/annexectomy) can be performed, which can be done laparoscopically or laparotomically depending on the patient&#8217;s medical history and the size of the uterus.<\/p>\n<p><!-- \/wp:paragraph --> <!-- wp:paragraph --><\/p>\n<p>In selected cases,<strong>uterine artery embolization <\/strong>can be performed, a minimally invasive procedure that selectively occludes the blood vessels supplying the uterus with embolizing substances under radiological control and regresses ischemic-based myomas.<\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Drug therapy<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p>It is mainly used to <strong>reduce anemia<\/strong> caused by profuse bleeding or <strong>prepare the patient for possible surgery.<\/strong><\/p>\n<p><strong data-start=\"215\" data-end=\"263\">Drugs for the treatment of uterine myomas<\/strong> include:<\/p>\n<ul>\n<li data-start=\"278\" data-end=\"464\">\n<p data-start=\"280\" data-end=\"325\"><strong data-start=\"280\" data-end=\"323\">Combined hormonal contraceptives (COCs):<\/strong><\/p>\n<ul data-start=\"328\" data-end=\"464\">\n<li data-start=\"328\" data-end=\"380\">\n<p data-start=\"330\" data-end=\"380\">Oral, vaginal or transdermal administration;<\/p>\n<\/li>\n<li data-start=\"383\" data-end=\"464\">\n<p data-start=\"385\" data-end=\"464\">help regulate the menstrual cycle and reduce profuse bleeding.<\/p>\n<\/li>\n<\/ul>\n<\/li>\n<li data-start=\"466\" data-end=\"680\">\n<p data-start=\"468\" data-end=\"492\"><strong data-start=\"468\" data-end=\"490\">Estro-progestins:<\/strong><\/p>\n<ul data-start=\"495\" data-end=\"680\">\n<li data-start=\"495\" data-end=\"534\">\n<p data-start=\"497\" data-end=\"534\">They combine estrogen and progestin;<\/p>\n<\/li>\n<li data-start=\"537\" data-end=\"603\">\n<p data-start=\"539\" data-end=\"603\">can be taken orally, vaginally or transdermally;<\/p>\n<\/li>\n<li data-start=\"606\" data-end=\"680\">\n<p data-start=\"608\" data-end=\"680\">serve to control menstrual symptoms and excessive bleeding.<\/p>\n<\/li>\n<\/ul>\n<\/li>\n<li data-start=\"682\" data-end=\"858\">\n<p data-start=\"684\" data-end=\"702\"><strong data-start=\"684\" data-end=\"700\">Progestins:<\/strong><\/p>\n<ul data-start=\"705\" data-end=\"858\">\n<li data-start=\"705\" data-end=\"799\">\n<p data-start=\"707\" data-end=\"799\">Available in oral, intrauterine (slow-release coil) or subcutaneous formulations;<\/p>\n<\/li>\n<li data-start=\"802\" data-end=\"858\">\n<p data-start=\"804\" data-end=\"858\">Can reduce bleeding and pelvic pain.<\/p>\n<\/li>\n<\/ul>\n<\/li>\n<li data-start=\"860\" data-end=\"1190\">\n<p data-start=\"862\" data-end=\"885\"><strong data-start=\"862\" data-end=\"883\">GnRH analogues:<\/strong><\/p>\n<ul data-start=\"888\" data-end=\"1190\">\n<li data-start=\"888\" data-end=\"941\">\n<p data-start=\"890\" data-end=\"941\">Inhibit the gonadotropic activity of the pituitary gland;<\/p>\n<\/li>\n<li data-start=\"944\" data-end=\"1062\">\n<p data-start=\"946\" data-end=\"1062\">temporarily suppress ovarian hormone production, creating a reversible state of <strong data-start=\"1039\" data-end=\"1059\">pseudo-menopause<\/strong>;<\/p>\n<\/li>\n<li data-start=\"1065\" data-end=\"1190\">\n<p data-start=\"1067\" data-end=\"1190\">mainly used to reduce the size of myomas before surgery or to manage severe symptoms.<\/p>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul class=\"wp-block-list\"><!-- \/wp:list-item --><\/ul>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t<\/div>\n\n\t<\/div>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"interdisciplinary-group\" class=\"h2_anchor\"><\/span>Interdisciplinary Group<\/h2>\n\n\n\n<p>Every cancer requires, in all phases of disease management, a<strong> multidisciplinary approach<\/strong> that at the Candiolo Institute is guaranteed by a team of different specialists, belonging to the various clinical and surgical departments of the Institute: this team is called GIC (Interdisciplinary Care Group). The GIC ensures that<strong> each patient is taken care of throughout the diagnostic-therapeutic process<\/strong>, including prescribing and booking examinations and communicating with the patient and his or her family members. The GIC defines and shares a <strong>personalized care pathway<\/strong> for each patient, based not only on the type and stage of the tumor, but also on the patient&#8217;s own characteristics.
