{"id":5270,"date":"2024-12-11T17:08:34","date_gmt":"2024-12-11T16:08:34","guid":{"rendered":"https:\/\/irccs.com\/patologie\/tumors-of-the-ovary\/"},"modified":"2026-06-10T17:10:36","modified_gmt":"2026-06-10T15:10:36","slug":"tumors-of-the-ovary","status":"publish","type":"pathology","link":"https:\/\/irccs.com\/en\/pathology\/tumors-of-the-ovary\/","title":{"rendered":"Tumors of the Ovary"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\"><span  id=\"pathology\" class=\"h2_anchor\"><\/span>Pathology<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Ovarian cancer is a <strong>malignant tumor affecting the ovaries<\/strong>, organs of the female reproductive system responsible for producing <strong>oocytes<\/strong> and secreting female sex hormones such as <strong>estrogen and progesterone<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ovarian cancer arises from the <strong>uncontrolled proliferation of ovarian cells<\/strong>. In most cases, it originates from <strong>epithelial cells lining the ovary<\/strong>, although it may also arise, more rarely, from <strong>germ cells or sex cord\u2013stromal cells<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is often an <strong>insidious disease<\/strong> because in its early stages it may not produce <strong>specific symptoms<\/strong>; for this reason, it is frequently diagnosed at an <strong>advanced stage<\/strong>. However, when detected early, ovarian cancer can be treated more effectively.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">The Numbers in Italy<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In 2024, according to the <strong><a href=\"https:\/\/www.registri-tumori.it\/cms\/pubblicazioni\/i-numeri-del-cancro-italia-2023\">AIRTUM<\/a><\/strong> (Italian Association of Cancer Registries), ovarian cancer in Italy recorded approximately <strong>5,423 new diagnoses<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Types<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Ovarian cancer can develop from different cellular components of the ovary.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Epithelial Tumors<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">These are the most common (<strong>about 90% of cases<\/strong>) and originate from the <strong>ovarian surface epithelium<\/strong>:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Type I: <strong>low-grade serous, mucinous, endometrioid, and clear cell tumors<\/strong><\/li>\n\n\n\n<li>Type II: <strong>high-grade serous tumors<\/strong>, which are the most frequent and clinically aggressive<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Borderline Tumors<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Tumors with <strong>low malignant potential<\/strong>, often diagnosed at <strong>early stages<\/strong> and in <strong>younger patients<\/strong>. They generally have a <strong>favorable prognosis<\/strong> and can often be managed with <strong>conservative surgery<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Stromal or Sex Cord Tumors<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Rare tumors arising from the <strong>ovarian connective tissue<\/strong> and capable of <strong>hormone production<\/strong>. They include <strong>granulosa cell tumors, thecoma-fibroma tumors, and Sertoli\u2013Leydig cell tumors<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Germ Cell Tumors<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Rare (<strong>approximately 5% of cases<\/strong>), originating from <strong>oocyte precursor cells<\/strong>. They occur most commonly in <strong>women under 30 years of age<\/strong>. Main types include <strong>teratomas, dysgerminomas, yolk sac tumors, and choriocarcinomas<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"symptoms\" class=\"h2_anchor\"><\/span>Symptoms<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Ovarian cIn early stages, ovarian cancer often causes <strong>no specific symptoms<\/strong>, making <strong>early diagnosis difficult<\/strong>. When present, symptoms may include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Abdominal bloating<\/strong> or persistent feeling of fullness (meteorism)<\/li>\n\n\n\n<li><strong>Increased urinary frequency<\/strong><\/li>\n\n\n\n<li><strong>Abdominal or pelvic pain<\/strong><\/li>\n\n\n\n<li><strong>Vaginal bleeding<\/strong><\/li>\n\n\n\n<li><strong>Constipation or diarrhea<\/strong><\/li>\n\n\n\n<li><strong>Unexplained fatigue<\/strong><\/li>\n\n\n\n<li><strong>Nausea or loss of appetite<\/strong> (more common in advanced stages)<\/li>\n\n\n\n<li><strong>Early satiety<\/strong> (feeling full shortly after starting a meal)<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">If these symptoms occur <strong>together or persist over time<\/strong>, <strong>gynecological evaluation<\/strong> is recommended.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"risk-factors\" class=\"h2_anchor\"><\/span>Risk Factors<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The main risk factors for ovarian cancer include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Early menarche or late menopause<\/strong><\/li>\n\n\n\n<li><strong>First pregnancy after the age of 35<\/strong><\/li>\n\n\n\n<li><strong>Pelvic inflammatory disease<\/strong><\/li>\n\n\n\n<li><strong>Nulliparity (having no children)<\/strong><\/li>\n\n\n\n<li><strong>Family history of ovarian or breast cancer<\/strong><\/li>\n\n\n\n<li><strong>Genetic mutations (BRCA1\/BRCA2, Lynch syndrome)<\/strong><\/li>\n\n\n\n<li><strong><a href=\"https:\/\/irccs.com\/en\/pathology\/endometriosis\/\">Endometriosis<\/a><\/strong><\/li>\n\n\n\n<li><strong>Polycystic ovary syndrome (PCOS)<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Protective factors include <strong>breastfeeding<\/strong>, <strong>younger age at first pregnancy (&lt;25 years)<\/strong>, and the use of <strong>oral contraceptives<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"diagnosis-and-examination\" class=\"h2_anchor\"><\/span>Diagnosis and Examination<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">To date, there are <strong>no diagnostic procedures with sufficient sensitivity and specificity<\/strong> for effective early detection; therefore, there is <strong>no established screening program<\/strong> for malignant ovarian cancer.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In cases of clinical suspicion, second-level investigations are usually performed, including:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Serum tumor markers (<strong>CA-125, CA19-9, CA15-3, CEA, alpha-fetoprotein<\/strong>)<\/li>\n\n\n\n<li><strong>Level 2 transvaginal ultrasound<\/strong> performed by an experienced operator<\/li>\n\n\n\n<li><strong>Contrast-enhanced CT scan<\/strong> of the chest, abdomen, and pelvis<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">If further evaluation is required, <strong>pelvic MRI<\/strong> and\/or <strong>total-body PET scanning<\/strong> may also be performed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A definitive diagnosis is established only through <strong>histological examination of surgical specimens<\/strong>.<\/p>\n\n\n\t<div id=\"block_6a297e6bd13f5\" class=\"external-video-block acf-block\">\n\t\t\n\n\n\n\t<\/div>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"therapies\" class=\"h2_anchor\"><\/span>Therapies<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong><a href=\"https:\/\/irccs.com\/en\/medical-area\/ovarian-cancer-center\/\">Ovarian Cancer Center<\/a> <\/strong>is a highly specialized referral unit offering a <strong>multidisciplinary approach<\/strong> to patients with ovarian cancer. Treatment is <strong>individualized<\/strong> and includes both surgical and medical (chemotherapy) strategies.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In selected cases, particularly in patients who wish to preserve fertility, <strong>fertility-sparing surgery<\/strong> may be considered. This involves removal of the tumor while preserving part of the <strong>ovary and\/or uterus<\/strong>, when oncologically safe, to maintain future fertility potential. The decision is made by a <strong>multidisciplinary team<\/strong> based on <strong>tumor stage, histological type, and oncological safety<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Medical treatment, administered before or after surgery, includes <strong>systemic chemotherapy<\/strong> and <strong>maintenance therapies<\/strong>. These may involve targeted biological agents such as <strong>monoclonal antibodies<\/strong> and oral therapies, including <strong>PARP inhibitors<\/strong>. The choice of therapy is guided by <strong>molecular and genetic profiling of the tumor<\/strong>.<\/p>\n\n\n\t<div id=\"block_6a297e6bd1452\" class=\"accordion-block acf-block\">\n\t\t<div class=\"my-4 lg:my-6\">\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Early Stage Ovarian Cancer<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p data-start=\"30\" data-end=\"118\">When the disease is <strong data-start=\"50\" data-end=\"84\">limited to one or both ovaries<\/strong>, surgical management may include:<\/p>\n<ul data-start=\"120\" data-end=\"712\">\n<li data-section-id=\"7s24tn\" data-start=\"120\" data-end=\"365\"><strong data-start=\"122\" data-end=\"191\">Bilateral hysterectomy with bilateral salpingo-oophorectomy (BSO)<\/strong>: removal of the <strong data-start=\"208\" data-end=\"253\">uterus, both ovaries, and fallopian tubes<\/strong> in order to eliminate all potential sites of disease spread or recurrence within the female reproductive tract.<\/li>\n<li data-section-id=\"14w0b42\" data-start=\"367\" data-end=\"513\"><strong data-start=\"369\" data-end=\"411\">Peritoneal staging (e.g., omentectomy)<\/strong>: removal of the <strong data-start=\"428\" data-end=\"439\">omentum<\/strong>, an abdominal fatty tissue that may harbor <strong data-start=\"483\" data-end=\"512\">occult metastatic disease<\/strong>.<\/li>\n<li data-section-id=\"14rzmls\" data-start=\"515\" data-end=\"712\"><strong data-start=\"517\" data-end=\"585\">Retroperitoneal staging (pelvic and para-aortic lymphadenectomy)<\/strong>: removal of <strong data-start=\"598\" data-end=\"651\">lymph nodes in the pelvic and para-aortic regions<\/strong> to assess whether the disease has spread beyond the ovaries.<\/li>\n<\/ul>\n<h3 data-section-id=\"1ldw846\" data-start=\"719\" data-end=\"738\">Surgical access<\/h3>\n<ul data-start=\"739\" data-end=\"1097\" data-is-last-node=\"\" data-is-only-node=\"\">\n<li data-section-id=\"10jt52d\" data-start=\"739\" data-end=\"876\"><strong data-start=\"741\" data-end=\"755\">Laparotomy<\/strong>: open surgical approach involving a <strong data-start=\"792\" data-end=\"820\">large abdominal incision<\/strong>, allowing direct access to pelvic and abdominal organs.<\/li>\n<li data-section-id=\"z4d3a5\" data-start=\"878\" data-end=\"1097\" data-is-last-node=\"\"><strong data-start=\"880\" data-end=\"905\">Operative laparoscopy<\/strong>: <strong data-start=\"907\" data-end=\"948\">minimally invasive surgical technique<\/strong> performed through small incisions (typically ~<strong data-start=\"995\" data-end=\"1003\">5 mm<\/strong>), associated with <strong data-start=\"1022\" data-end=\"1096\">reduced postoperative pain, shorter hospital stay, and faster recovery<\/strong>.<\/li>\n<\/ul>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Advanced Ovarian Cancer<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p data-start=\"74\" data-end=\"181\">When the disease extends beyond the ovaries, the diagnostic and therapeutic pathway involves several steps:<\/p>\n<ul>\n<li><strong data-start=\"185\" data-end=\"211\">Diagnostic Laparoscopy<\/strong>: performed to directly assess <strong data-start=\"242\" data-end=\"274\">intra-abdominal tumor spread<\/strong>, avoiding reliance solely on radiological imaging. It allows visualization of <strong data-start=\"353\" data-end=\"375\">tumor distribution<\/strong> and enables <strong data-start=\"388\" data-end=\"407\" data-is-only-node=\"\">biopsy sampling<\/strong> for immediate histological examination, supporting intraoperative diagnostic confirmation.<\/li>\n<li><strong data-start=\"502\" data-end=\"533\">Cytoreducibility Assessment<\/strong>: evaluation of the feasibility of optimal debulking using scoring systems such as the <strong data-start=\"620\" data-end=\"652\">Predictive Index Value (PIV)<\/strong> or <strong data-start=\"656\" data-end=\"673\">Fagotti score<\/strong>. This determines the likelihood of achieving <strong data-start=\"719\" data-end=\"744\" data-is-only-node=\"\">optimal cytoreduction<\/strong> (complete macroscopic tumor removal or residual disease &lt; <strong data-start=\"803\" data-end=\"811\">1 cm<\/strong>).<\/li>\n<li>If the score is favorable, <strong data-start=\"844\" data-end=\"879\">Primary Debulking Surgery (PDS)<\/strong> is performed in the same operative session. <strong data-start=\"924\" data-end=\"949\">Cytoreductive surgery<\/strong> involves removal of all <strong data-start=\"974\" data-end=\"1007\">macroscopically visible tumor<\/strong>, including the <strong data-start=\"1023\" data-end=\"1051\">uterus, ovaries, omentum<\/strong>, and <strong data-start=\"1057\" data-end=\"1096\">pathologically enlarged lymph nodes<\/strong>, as well as any other involved tissues or organs.<\/li>\n<li>If the tumor is not considered resectable, <strong data-start=\"1193\" data-end=\"1214\">targeted biopsies<\/strong> are performed to define the <strong data-start=\"1243\" data-end=\"1291\">histological, molecular, and genetic profile<\/strong> of the disease. Patients are then started on <strong data-start=\"1337\" data-end=\"1365\">neoadjuvant chemotherapy<\/strong> to reduce tumor burden.
