{"id":5313,"date":"2024-12-11T11:43:56","date_gmt":"2024-12-11T10:43:56","guid":{"rendered":"https:\/\/irccs.com\/patologie\/bladder-tumors\/"},"modified":"2026-06-11T16:22:46","modified_gmt":"2026-06-11T14:22:46","slug":"bladder-tumors","status":"publish","type":"pathology","link":"https:\/\/irccs.com\/en\/pathology\/bladder-tumors\/","title":{"rendered":"Bladder Tumors"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\"><span  id=\"pathology\" class=\"h2_anchor\"><\/span>Pathology<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Bladder cancer<\/strong> results from the <strong>malignant transformation of cells in the bladder<\/strong>, the organ that <strong>stores urine before it is expelled from the body<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In <strong>90% of cases<\/strong>, it is a <strong>urothelial carcinoma<\/strong>, meaning it originates from the <strong>cells of the urothelium<\/strong>, the tissue that <strong>lines the inside of the urinary tract<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This cancer is <strong>strongly associated with cigarette smoking<\/strong>, occurs most frequently between the ages of <strong>60 and 70<\/strong>, and affects <strong>men nearly four times more often than women<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">The Numbers in Italy<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Bladder cancer<\/strong> accounts for approximately <strong>7% of all newly diagnosed cancers<\/strong>, making it the <strong>second most common urological malignancy after prostate cancer<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">According to the <strong>2023<\/strong> <strong><a href=\"https:\/\/www.registri-tumori.it\/cms\/pubblicazioni\/i-numeri-del-cancro-italia-2023\">AIRTUM registry<\/a><\/strong> (Italian Association of Cancer Registries) <strong>report<\/strong>, approximately <strong>31,016 new cases of bladder cancer<\/strong> were diagnosed in Italy, including <strong>25,227 men<\/strong> and <strong>5,789 women<\/strong>. These figures represent <strong>11.4% of all newly diagnosed cancers in men<\/strong> and <strong>3.2% in women<\/strong>, respectively.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Types<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Transitional cell urothelial carcinoma<\/strong> accounts for approximately <strong>90% of all bladder cancers<\/strong>. It may present as a <strong>superficial tumor<\/strong> (about <strong>two-thirds of cases<\/strong>) or as an <strong>infiltrating form<\/strong> (<strong>muscle-invasive disease<\/strong>).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Other, less common histological types include <strong>adenocarcinoma<\/strong> and <strong>primary squamous cell carcinoma<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong>clinical behavior of bladder cancer<\/strong> is highly variable, with significant differences in <strong>aggressiveness<\/strong>, <strong>risk of recurrence<\/strong>, and <strong>potential for metastatic spread<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"symptoms\" class=\"h2_anchor\"><\/span>Symptoms<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Bladder cancer<\/strong> may present with symptoms that are also common to other <strong>urinary tract disorders<\/strong>. The most frequent sign is <strong>blood in the urine (hematuria)<\/strong>, which may be accompanied by <strong>blood clots<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Other possible symptoms include <strong>burning during urination<\/strong>, <strong>pain in the bladder region<\/strong>, <strong>difficulty or pain while urinating<\/strong>, and <strong>increased susceptibility to urinary tract infections<\/strong>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"risk-factors\" class=\"h2_anchor\"><\/span>Risk Factors<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Risk Factors<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The main <strong>risk factors for bladder cancer<\/strong> include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Cigarette smoking<\/strong><\/li>\n\n\n\n<li><strong>Prolonged exposure to chemicals<\/strong>, particularly <strong>aromatic amines<\/strong> and <strong>nitrosamines<\/strong>, commonly found in the <strong>textile, dye, rubber, and leather industries<\/strong><\/li>\n\n\n\n<li><strong>Previous radiotherapy to the pelvic region<\/strong><\/li>\n\n\n\n<li>Use of certain medications, including <strong>cyclophosphamide<\/strong> and <strong>ifosfamide<\/strong><\/li>\n\n\n\n<li><strong>Parasitic infections<\/strong>, particularly <strong>Schistosoma haematobium<\/strong>, which is prevalent in some regions, especially <strong>Egypt<\/strong><\/li>\n\n\n\n<li><strong>Dietary factors<\/strong>, with a <strong>high intake of fried foods and fats<\/strong> associated with an increased risk<\/li>\n\n\n\n<li><strong>Genetic predisposition<\/strong>, which may contribute to the <strong>development of the disease<\/strong><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"diagnosis-and-evaluation\" class=\"h2_anchor\"><\/span>Diagnosis and Evaluation<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong>diagnostic pathway for bladder cancer<\/strong> typically begins with a visit to a <strong>primary care physician<\/strong>, prompted by the most common presenting symptom: <strong>blood in the urine (hematuria)<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">To investigate the cause, the physician may order <strong>blood tests<\/strong> and <strong>urine cytology<\/strong>, a test that examines urine samples for the presence of <strong>cancer cells<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The next step is a <strong>specialist consultation with a urologist<\/strong>, who will assess the findings and determine which <strong>additional investigations<\/strong> are required to complete the diagnostic workup.<\/p>\n\n\n\t<div id=\"block_6a2ac4b4c8b31\" class=\"accordion-block acf-block\">\n\t\t<div class=\"my-4 lg:my-6\">\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Ultrasound<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p data-start=\"20\" data-end=\"199\"><strong data-start=\"20\" data-end=\"34\">Ultrasound<\/strong> is a diagnostic imaging technique that uses <strong data-start=\"79\" data-end=\"107\">sound waves (ultrasound)<\/strong> emitted by a special probe to produce real-time images of internal structures on a monitor.<\/p>\n<p data-start=\"201\" data-end=\"306\">It is a <strong data-start=\"209\" data-end=\"245\">simple, non-invasive examination<\/strong> performed by placing the probe on the patient&#8217;s <strong data-start=\"294\" data-end=\"305\">abdomen<\/strong>.<\/p>\n<p data-start=\"308\" data-end=\"572\">Ultrasound enables the assessment of the <strong data-start=\"349\" data-end=\"387\">size and morphology of the bladder<\/strong> and can help distinguish between <strong data-start=\"421\" data-end=\"453\">benign and malignant lesions<\/strong> with good reliability. To obtain optimal images, the <strong data-start=\"507\" data-end=\"571\">bladder should be well distended and therefore full of urine<\/strong>.<\/p>\n<p data-start=\"574\" data-end=\"682\" data-is-last-node=\"\" data-is-only-node=\"\">The examination is typically performed in a <strong data-start=\"618\" data-end=\"642\">Radiology Department<\/strong> and usually takes <strong data-start=\"661\" data-end=\"681\">about 10 minutes<\/strong>.<\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Computed Tomography (CT)<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p data-start=\"34\" data-end=\"197\"><strong data-start=\"34\" data-end=\"62\">Computed Tomography (CT)<\/strong> is an imaging technique that uses <strong data-start=\"97\" data-end=\"107\">X-rays<\/strong> to generate <strong data-start=\"120\" data-end=\"165\">highly detailed, three-dimensional images<\/strong> of different parts of the body.<\/p>\n<p data-start=\"199\" data-end=\"455\">In the evaluation of <strong data-start=\"220\" data-end=\"238\">bladder cancer<\/strong>, CT is used to determine the <strong data-start=\"268\" data-end=\"291\">presence of a tumor<\/strong> and to assess whether it has <strong data-start=\"321\" data-end=\"376\">spread to nearby lymph nodes or adjacent structures<\/strong>, including the <strong data-start=\"392\" data-end=\"403\">ureters<\/strong>, the tubes that connect the kidneys to the bladder.<\/p>\n<p data-start=\"457\" data-end=\"589\">The examination requires the administration of an <strong data-start=\"507\" data-end=\"537\">intravenous contrast agent<\/strong> and is typically performed <strong data-start=\"565\" data-end=\"588\">on an empty stomach<\/strong>.<\/p>\n<p data-start=\"591\" data-end=\"803\" data-is-last-node=\"\" data-is-only-node=\"\">CT scans are usually carried out in a <a href=\"https:\/\/irccs.com\/en\/medical-area\/radiodiagnostics\/\"><strong data-start=\"629\" data-end=\"653\">Radiology Department<\/strong> <\/a>and take approximately <strong data-start=\"677\" data-end=\"691\">15 minutes<\/strong>. Grading ranges from <strong>Grade 1 to Grade 3<\/strong> and provides information about the <strong>biological aggressiveness<\/strong> of the tumor.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Grade 1 (low grade):<\/strong> Cancer cells closely resemble normal cells and tend to <strong>grow and spread slowly<\/strong>.<\/li>\n\n\n\n<li><strong>Grade 2 (intermediate grade):<\/strong> Cancer cells show a <strong>moderate degree of abnormality<\/strong> and have an intermediate growth potential.<\/li>\n\n\n\n<li><strong>Grade 3 (high grade):<\/strong> Cancer cells appear <strong>highly abnormal<\/strong>, are generally <strong>more aggressive<\/strong>, and are more likely to <strong>grow rapidly, recur, or spread<\/strong>.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"therapies\" class=\"h2_anchor\"><\/span>Therapies<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">After diagnosis, the <strong><a href=\"https:\/\/irccs.com\/en\/pathology\/bladder-tumors\/#multi-disciplinary-team\">Multidisciplinary Team (GIC)<\/a><\/strong> carefully evaluates several factors, including the <strong>stage and aggressiveness of the tumor<\/strong>, as well as the patient&#8217;s <strong>age and overall health status<\/strong>, to develop an <strong>individualized treatment plan<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For <strong>superficial (non-muscle-invasive) tumors<\/strong>, the primary treatment is <strong>transurethral resection of the bladder tumor (TURBT)<\/strong>. This procedure is often followed by <strong>intravesical chemotherapy<\/strong> or <strong>immunotherapy<\/strong> to reduce the risk of recurrence.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For <strong>muscle-invasive tumors<\/strong>, which are more aggressive and involve the deeper layers of the bladder wall, <strong>endoscopic surgery alone is insufficient<\/strong>. Treatment typically begins with <strong>chemotherapy or immunotherapy<\/strong> to reduce the extent of the disease, followed approximately <strong>three months later<\/strong> by either <strong>radical cystectomy (surgical removal of the bladder)<\/strong> or <strong>radiation therapy<\/strong>, depending on the clinical situation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For <strong>advanced or metastatic bladder cancer<\/strong>, treatment options may include <strong>chemotherapy<\/strong>, <strong>immunotherapy<\/strong>, and other therapies aimed at <strong>controlling symptoms and improving quality of life<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In selected patients with <strong>particularly aggressive tumors<\/strong> that do not respond to standard treatments, participation in <strong>clinical trials<\/strong> may provide access to <strong>experimental therapies<\/strong>. A thin tubular instrument equipped with optical fibers is inserted through the <strong data-start=\"726\" data-end=\"737\">urethra<\/strong>, allowing the surgeon to visualize the tumor and remove it from the bladder lining using an <strong data-start=\"830\" data-end=\"860\">electrosurgical instrument<\/strong>.<\/p>\n<p data-start=\"863\" data-end=\"1102\">Following the procedure, a <strong data-start=\"890\" data-end=\"910\">urinary catheter<\/strong> is usually placed and remains in position for several days. Hospital discharge generally occurs within <strong data-start=\"1014\" data-end=\"1026\">48 hours<\/strong>, and most patients can resume normal physical activities after a few weeks.<\/p>\n<h3 data-section-id=\"1goxeqq\" data-start=\"1104\" data-end=\"1130\"><span role=\"text\"><strong data-start=\"1108\" data-end=\"1130\">Radical Cystectomy<\/strong><\/span><\/h3>\n<p data-start=\"1132\" data-end=\"1251\"><strong data-start=\"1132\" data-end=\"1154\">Radical cystectomy<\/strong> involves the <strong data-start=\"1168\" data-end=\"1203\">complete removal of the bladder<\/strong>, together with the surrounding <strong data-start=\"1235\" data-end=\"1250\">lymph nodes<\/strong>.<\/p>\n<p data-start=\"1253\" data-end=\"1684\">When appropriate, the procedure can be performed using a <strong data-start=\"1310\" data-end=\"1358\">minimally invasive robotic-assisted approach<\/strong>. Through several small abdominal incisions, laparoscopic instruments are inserted and controlled by the surgeon from a dedicated console. This approach aims to <strong data-start=\"490\" data-end=\"576\">eliminate any remaining cancer cells within the bladder while preserving the organ<\/strong>.<\/p>\n<p data-start=\"579\" data-end=\"954\">Radiation therapy uses a machine that delivers <strong data-start=\"626\" data-end=\"661\">highly targeted radiation beams<\/strong> from outside the body directly to the bladder. Treatment is performed on an <strong data-start=\"738\" data-end=\"758\">outpatient basis<\/strong>, usually <strong data-start=\"768\" data-end=\"809\">five days per week (Monday to Friday)<\/strong> for <strong data-start=\"814\" data-end=\"830\">4 to 7 weeks<\/strong>. Each session lasts approximately <strong data-start=\"865\" data-end=\"885\">10 to 30 minutes<\/strong>, although the actual delivery of radiation takes only a few minutes.<\/p>\n<p data-start=\"956\" data-end=\"1129\">Advanced radiation therapy technologies allow treatment to be delivered with <strong data-start=\"1033\" data-end=\"1054\">greater precision<\/strong>, <strong data-start=\"1056\" data-end=\"1083\">shorter treatment times<\/strong>, and <strong data-start=\"1089\" data-end=\"1113\">reduced side effects<\/strong>. When instilled directly into the bladder, BCG stimulates the patient&#8217;s immune system to attack cancer cells.<\/p>\n<p data-start=\"475\" data-end=\"788\">BCG may be administered after <strong data-start=\"505\" data-end=\"561\">transurethral resection of the bladder tumor (TURBT)<\/strong>, often in combination with other treatments, to <strong data-start=\"610\" data-end=\"643\">reduce the risk of recurrence<\/strong>. In selected cases, it may also be used as part of a treatment strategy aimed at reducing the burden of disease before more extensive treatment.<\/p>\n<p data-start=\"790\" data-end=\"1114\">The treatment is delivered directly into the bladder through a <strong data-start=\"853\" data-end=\"873\">urinary catheter<\/strong>. The solution is retained in the bladder for several hours before being naturally expelled. Instillations are typically performed at <strong data-start=\"1007\" data-end=\"1038\">weekly or monthly intervals<\/strong> and may continue for <strong data-start=\"1060\" data-end=\"1076\">1 to 3 years<\/strong>, depending on the treatment protocol.<\/p>\n<p data-start=\"1116\" data-end=\"1253\">For <strong data-start=\"1120\" data-end=\"1161\">advanced or metastatic bladder cancer<\/strong>, other forms of immunotherapy are available and are usually administered <strong data-start=\"1235\" data-end=\"1252\">intravenously<\/strong>.<\/p>\n<p data-start=\"1255\" data-end=\"1477\" data-is-last-node=\"\" data-is-only-node=\"\">Among these, <strong data-start=\"1268\" data-end=\"1285\">Pembrolizumab<\/strong> has shown promising results. Because these drugs also affect some healthy cells that divide quickly, treatment can cause <strong data-start=\"242\" data-end=\"258\">side effects<\/strong>, which generally improve or resolve after therapy is completed.<\/p>\n<p data-start=\"324\" data-end=\"596\">For <strong data-start=\"328\" data-end=\"380\">superficial (non-muscle-invasive) bladder cancer<\/strong>, chemotherapy is often used after <strong data-start=\"415\" data-end=\"471\">transurethral resection of the bladder tumor (TURBT)<\/strong>, sometimes in combination with <strong data-start=\"503\" data-end=\"520\">immunotherapy<\/strong>, to eliminate any remaining cancer cells and reduce the risk of recurrence.<\/p>\n<p data-start=\"598\" data-end=\"834\">In these cases, chemotherapy is administered by <strong data-start=\"646\" data-end=\"675\">intravesical instillation<\/strong>. The drug, in liquid form, is delivered directly into the bladder through a <strong data-start=\"752\" data-end=\"772\">urinary catheter<\/strong> and retained for at least <strong data-start=\"799\" data-end=\"811\">one hour<\/strong> before being expelled.<\/p>\n<p data-start=\"836\" data-end=\"1077\">Treatment typically consists of <strong data-start=\"868\" data-end=\"894\">multiple instillations<\/strong>, initially performed at <strong data-start=\"919\" data-end=\"939\">weekly intervals<\/strong> and later at <strong data-start=\"953\" data-end=\"974\">monthly intervals<\/strong>, although the schedule may vary depending on the characteristics of the tumor and the medication used.<\/p>\n<p data-start=\"1079\" data-end=\"1191\">The procedure is performed on an <strong data-start=\"1112\" data-end=\"1132\">outpatient basis<\/strong>, is relatively quick, and is generally <strong data-start=\"1172\" data-end=\"1190\">well tolerated<\/strong>.<\/p>\n<p data-start=\"1193\" data-end=\"1361\">For <strong data-start=\"1197\" data-end=\"1231\">muscle-invasive bladder cancer<\/strong>, chemotherapy is usually administered <strong data-start=\"1270\" data-end=\"1287\">intravenously<\/strong>.
