The thyroid has a characteristic <strong>butterfly shape<\/strong>: the two lateral lobes lie on either side of the larynx, while the central portion connecting them is known as the isthmus.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The thyroid gland <strong>produces hormones essential for regulating metabolism, heart rate, body temperature<\/strong>, and, <strong>in children, normal physical and cognitive development.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Thyroid cancer most commonly<strong> affects women <\/strong>between the <strong>ages of 40 and 60 <\/strong>and represents the <strong>second most frequent malignancy in women under 50<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">The Numbers in Italy<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">According to data from the <strong><a href=\"https:\/\/www.registri-tumori.it\/cms\/pubblicazioni\/i-numeri-del-cancro-italia-2023\">AIRTUM registry,<\/a><\/strong> approximately <strong>11,378 new cases of thyroid cancer<\/strong> were diagnosed in Italy in 2024, including <strong>8,322 women and 3,056 men<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The incidence of thyroid cancer has <strong>increased in recent decades<\/strong>, largely due to the more widespread use of diagnostic ultrasound. This has led to the <strong>detection of very small thyroid nodules<\/strong>, some of which may be clinically indolent and not clinically significant.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Types<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Thyroid cancer includes several histological subtypes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Well-differentiated forms (papillary and follicular):<\/strong> approximately 85\u201390% of cases<\/li>\n\n\n\n<li><strong>Poorly differentiated carcinoma:<\/strong> approximately 5\u20137% of cases<\/li>\n\n\n\n<li><strong>Medullary carcinoma:<\/strong> approximately 5\u20137% of cases<\/li>\n\n\n\n<li><strong>Undifferentiated (anaplastic) carcinoma:<\/strong> approximately 2\u20133% of cases; the most aggressive form<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">In addition to traditional histological classification, current clinical practice increasingly considers the molecular characteristics of tumors, which can help guide more personalized treatment strategies. <strong>Prognosis depends on both tumor type and the presence of specific molecular alterations.<\/strong> Well-differentiated forms generally have an excellent prognosis, whereas poorly differentiated, medullary, and anaplastic forms require more complex and individualized therapeutic approaches.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"symptoms\" class=\"h2_anchor\"><\/span>Symptoms<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Thyroid nodules typically do not affect thyroid function, and <strong>many patients remain asymptomatic.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The most common clinical finding is the presence of a <strong>palpable nodule in the thyroid gland<\/strong>, detected in the anterior neck region. It is important to emphasize that the majority of thyroid nodules are benign; <strong>only about 5\u201310% correspond to malignant tumors.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In rarer cases, particularly in more aggressive forms, the disease may present as a rapidly enlarging neck mass involving the thyroid gland and laterocervical lymph nodes. Even in less aggressive tumors, lymph node involvement may occur; therefore, careful ultrasound evaluation is essential, especially in younger patients.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"risk-factors\" class=\"h2_anchor\"><\/span>Risk Factors<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Thyroid cancer is <strong>more common in women<\/strong>, with a female-to-male ratio of approximately 4:1.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Known risk factors include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Iodine deficiency<\/strong>, which may lead to goiter (enlargement of the thyroid gland) and the development of multiple nodules, some of which may undergo malignant transformation<\/li>\n\n\n\n<li><strong>Exposure to ionizing radiation<\/strong>, particularly during childhood or adolescence, including prior radiotherapy to the neck or exposure to radioactive fallout. For this reason, unnecessary medical radiation exposure (e.g., CT scans or X-rays involving the neck region) should be carefully evaluated in younger patients<\/li>\n\n\n\n<li><strong>Family history<\/strong>, especially in hereditary forms of medullary thyroid carcinoma, which may occur as part of multiple endocrine neoplasia (MEN) syndromes. These are often associated with mutations in the <strong>RET oncogene<\/strong>, which can be inherited<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Family members of patients with medullary thyroid carcinoma are advised to undergo specialist evaluation and genetic testing to assess the presence of RET mutations associated with hereditary disease.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"diagnosis-and-evaluation\" class=\"h2_anchor\"><\/span>Diagnosis and Evaluation<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">At the <strong>Candiolo Cancer Institute<\/strong>, the diagnostic pathway for suspected thyroid cancer begins with a specialist clinical evaluation involving ENT surgeons, general surgeons, and endocrinologists.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">During the initial assessment, thyroid function tests are reviewed if already available, or prescribed if necessary. All patients undergo <strong>neck ultrasound <\/strong>as a first-line imaging examination. When indicated, further diagnostic investigations are performed, including advanced imaging studies and interventional procedures such as ultrasound-guided <strong>fine-needle aspiration biopsy <\/strong>(FNAB) to obtain cytological confirmation.<\/p>\n\n\n\t<div id=\"block_6a295ff7b0a47\" class=\"accordion-block acf-block\">\n\t\t<div class=\"my-4 lg:my-6\">\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Neck Ultrasound<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p data-start=\"21\" data-end=\"158\">Neck ultrasound is a noninvasive, readily available imaging examination that provides high-resolution visualization of the thyroid gland.<\/p>\n<p data-start=\"160\" data-end=\"184\">It allows clinicians to:<\/p>\n<ul data-start=\"186\" data-end=\"450\">\n<li data-section-id=\"1wmxjea\" data-start=\"186\" data-end=\"278\"><strong>Detect even very small thyroid nodules<\/strong> that cannot be identified on physical examination<\/li>\n<li data-section-id=\"1hfu429\" data-start=\"279\" data-end=\"335\"><strong>Assess nodule size and morphological characteristics<\/strong><\/li>\n<li data-section-id=\"i87pg\" data-start=\"336\" data-end=\"379\"><strong>Evaluate the surrounding thyroid tissue<\/strong><\/li>\n<li data-section-id=\"1c1sqs9\" data-start=\"380\" data-end=\"450\"><strong>Examine cervical lymph nodes<\/strong> for possible pathological involvement<\/li>\n<\/ul>\n<p data-start=\"452\" data-end=\"585\" data-is-last-node=\"\" data-is-only-node=\"\">Because of its <strong>accuracy<\/strong> and <strong>safety<\/strong>, neck ultrasound is a fundamental first-line tool in the diagnostic evaluation of thyroid disease.<\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Fibrolaryngoscopy<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p data-start=\"23\" data-end=\"197\">Fibrolaryngoscopy is an endoscopic examination that may be <strong>performed by the ENT<\/strong> specialist to <strong>assess vocal cord mobility<\/strong> and identify any structural or surface abnormalities.<\/p>\n<p data-start=\"199\" data-end=\"449\">The procedure is carried out <strong>using a flexible fiber-optic endoscope.<\/strong> This thin, flexible tube is gently inserted through the patient\u2019s nasal cavity, allowing the physician to obtain a direct and detailed view of the larynx and surrounding structures.<\/p>\n<p data-start=\"451\" data-end=\"671\" data-is-last-node=\"\" data-is-only-node=\"\">Fibrolaryngoscopy is performed in an <strong>outpatient<\/strong> ENT setting and is generally well tolerated. It can be conducted <strong>without anesthesia<\/strong> <strong>or<\/strong>, when necessary, with the application of <strong>local anesthesia<\/strong> to improve patient comfort.<\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Thyroid Scintigraphy<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p data-start=\"26\" data-end=\"253\">Thyroid scintigraphy is<strong> a nuclear medicine imaging technique<\/strong> used to evaluate the functional activity of one or more thyroid nodules and to help differentiate between hyperfunctioning (often benign) and non-functioning lesions.<\/p>\n<p data-start=\"255\" data-end=\"621\">The examination involves the intravenous administration of a small amount of radioactive tracer, typically <strong data-start=\"362\" data-end=\"402\">technetium-99m (99mTc) pertechnetate<\/strong>, which is taken up by thyroid tissue. The radiation emitted is detected by a gamma camera, which generates an image of the thyroid gland showing the distribution and functional activity of the tracer within the tissue.<\/p>\n<p data-start=\"623\" data-end=\"743\">This allows clinicians to distinguish different types of nodules based on their uptake patterns and functional behavior.<\/p>\n<p data-start=\"745\" data-end=\"900\" data-is-last-node=\"\" data-is-only-node=\"\">The procedure is simple, noninvasive, and painless. When malignancy is confirmed or suspected, the pathologist further characterizes the tumor type and its specific features.<\/p>\n<p data-start=\"404\" data-end=\"470\">Thyroid cancers are broadly classified into three main categories:<\/p>\n<ul data-start=\"472\" data-end=\"630\">\n<li data-section-id=\"1dnnrq\" data-start=\"472\" data-end=\"554\"><strong data-start=\"474\" data-end=\"502\">Differentiated carcinoma<\/strong>, which includes papillary and follicular subtypes<\/li>\n<li data-section-id=\"7c5y30\" data-start=\"555\" data-end=\"582\"><strong data-start=\"557\" data-end=\"580\">Medullary carcinoma<\/strong><\/li>\n<li data-section-id=\"1fe8lc3\" data-start=\"583\" data-end=\"630\"><strong data-start=\"585\" data-end=\"628\">Undifferentiated (anaplastic) carcinoma<\/strong><\/li>\n<\/ul>\n<p data-start=\"632\" data-end=\"915\">Papillary and follicular differentiated carcinomas are the most frequent, accounting for approximately 75% and 15% of thyroid cancer cases respectively, and are associated with a very favorable prognosis, with long-term survival rates reaching around 90% at 20 years after diagnosis.<\/p>\n<p data-start=\"917\" data-end=\"1206\">Medullary carcinoma represents approximately 5\u201310% of thyroid cancers.
