Initial symptoms, such as heartburn, nausea, or digestive difficulties, may be similar to less severe disorders, so diagnosis may occur in more advanced stages. However, it is important<strong> not to overlook symptoms <\/strong>and to seek timely referral to a specialist, because early diagnosis allows more effective treatments to be planned and improves the chances of cure.<\/p>\n\n<p>Gastric cancer is <strong>one of the most common cancers worldwide<\/strong>, with more than one million new cases diagnosed each year globally, and is the <strong>fifth most common malignancy by incidence<\/strong> in men and seventh in women.  <\/p>\n\n<h3 class=\"wp-block-heading\">The numbers in Italy<\/h3>\n\n<p>In Italy, according to the<a href=\"https:\/\/www.registri-tumori.it\/cms\/pubblicazioni\/i-numeri-del-cancro-italia-2023\"> AIRTUM <\/a>(Italian Association of Cancer Registries) registry, about 14,105 new diagnoses of stomach cancer were recorded in 2023, including 8,593 in men and 5,512 in women. In both sexes, this neoplasm occurs mostly in people of advanced adult age, around 60-70 years.<\/p>\n\n<h3 class=\"wp-block-heading\">Main forms of gastric cancer<\/h3>\n\n<p>Gastric cancer can occur in several forms, which differ in cellular characteristics and location:<\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong>adenocarcinoma<\/strong>: this is the most common form, accounting for about 90-95% of cases and originating from gastric mucosal cells;<\/li>\n\n\n\n<li><strong>Gastric lymphomas<\/strong>: rare tumors that develop in the lymphatic tissue of the stomach wall, often related to <em>Helicobacter pylori<\/em> infection;<\/li>\n\n\n\n<li><strong>Gastrointestinal stromal tumors (GISTs)<\/strong>: rare formations that develop from cells in the muscles or supporting tissue of the stomach;<\/li>\n\n\n\n<li><strong>Gastric carcinoids<\/strong>: tumors originating from neuroendocrine cells of the gastric mucosa, usually slow-growing.<\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\"><span  id=\"risk-factors\" class=\"h2_anchor\"><\/span>Risk factors<\/h2>\n\n<p>The main risk factors associated with the development of stomach cancer involve various<strong> conditions and lifestyle habits<\/strong>, including:<\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong>diet high in salty and smoked foods and low in fruits and vegetables<\/strong>, which can increase the risk of disease onset;<\/li>\n\n\n\n<li><strong>Cigarette smoke<\/strong>;<\/li>\n\n\n\n<li><strong>Family history <\/strong>of stomach cancer, which may indicate a genetic or environmental predisposition;<\/li>\n\n\n\n<li><strong>chronic inflammation of the stomach<\/strong>, particularly atrophic gastritis, often linked to infection with <em>Helicobacter pylori<\/em> (Hp), a bacterium known to increase the risk of gastric cancer;<\/li>\n\n\n\n<li><strong>pernicious anemia,<\/strong> a condition that can alter the gastric mucosa and promote the appearance of precancerous lesions;<\/li>\n\n\n\n<li><strong>presence of gastric polyps,<\/strong> some types of which can develop into cancer;<\/li>\n\n\n\n<li>previous <strong>gastric resection for ulcer<\/strong>, which can change the structure and function of the stomach, increasing the risk.<\/li>\n<\/ul>\n\n<p>A <strong>significant reduction in the incidence of stomach cancer<\/strong> has been observed in recent decades, in part attributable to the use of food refrigeration, which has reduced the need for traditional preservation techniques such as salting and smoking.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"diagnosis-and-examination\" class=\"h2_anchor\"><\/span>Diagnosis and examination<\/h2>\n\n<p>The diagnostic pathway for stomach cancer usually begins with a visit to the<strong> specialist grastroenterologist<\/strong>, a visit that the general practitioner may prescribe to ascertain the cause of general symptoms such as nausea, stomach pain, and bloating sensation.<\/p>\n\n<p>The gastroenterologist in turn, after examining the patient and gathering information about the patient&#8217;s health status, prescribes the necessary diagnostic tests to check for cancer. The routine examination is gastroscopy.<\/p>\n\t<div id=\"block_6931bb285c8c8\" class=\"accordion-block acf-block\">\n\t\t<div class=\"my-4 lg:my-6\">\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Gastroscopy<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p><span style=\"font-weight: 400;\">Gastroscopy is a <\/span><b>diagnostic technique<\/b><span style=\"font-weight: 400;\"> that allows the gastroenterologist to <\/span><b>visually explore the esophagus, stomach, and duodenum<\/b><span style=\"font-weight: 400;\"> and to identify exactly the site of the tumor.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It consists of inserting, through the mouth or nose down the esophagus to the stomach, the <\/span><b>gastroscope<\/b><span style=\"font-weight: 400;\">, a long, flexible probe equipped with a light source and a camera that transmits images to a computer screen.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The gastroscope also allows the gastroenterologist, using miniaturized surgical forceps inserted into the probe, to take one or more fragments of the suspected lesion and then perform a biopsy.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The procedure does not cause pain but can be uncomfortable, so it may be necessary to administer mild anesthesia to the patient, either by spraying it directly into the throat or injecting it intravenously. The examination takes about 5 to 10 minutes.<\/span><\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Ecoendoscopy<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p><span style=\"font-weight: 400;\"><a href=\"https:\/\/irccs.com\/aree-mediche\/gastroenterologia-ed-endoscopia-digestiva\/\">Echoendoscopy<\/a> is based on the use of a probe similar to the gastroscope but which additionally allows, through the use of ultrasound (like ultrasound), to <\/span><b>analyze the walls of the stomach and other organs in the abdomen and chest <\/b><span style=\"font-weight: 400;\">and to detect even very small lesions that would not be visible with a normal ultrasound scan from the outside.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Also with this examination, using a fine needle that is inserted through the endoscopic probe, cell and\/or tissue sampling (biopsies) can be performed.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The procedure takes longer than traditional gastroscopy (30-40 minutes) and is performed with sedation administered intravenously.<\/span><\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Computed Tomography (CT)<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p><span style=\"font-weight: 400;\">CT scan, a diagnostic technique that uses X-rays to create very detailed, three-dimensional images of various areas of the body, is used to <\/span><b>precisely locate the tumor in the stomach and check whether it has spread to the lymph nodes and other organs <\/b><span style=\"font-weight: 400;\">nearby in the abdomen, chest and neck.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The examination requires a contrast agent that is injected into the vein. It is performed on an empty stomach at the Division of Radiodiagnostics and lasts about a quarter of an hour, during which time the patient should remain as still as possible.<\/span><\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Histological and molecular examination<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p><span style=\"font-weight: 400;\">By microscopically analyzing the tissue and cells taken in biopsies, <\/span><b>the anatomopathologist first defines the histotype of the tumor<\/b><span style=\"font-weight: 400;\">, that is, the type of cells of which it is made up. The most common type of stomach cancer (about 90-95% of all cases) is the<\/span><b>adenocarcinoma<\/b><span style=\"font-weight: 400;\">: This tumor develops from the glandular cells that make up the inner mucosa of the stomach and whose function is to secrete substances necessary for the digestion of food.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The anatomo-pathologist <\/span><b>also determines the grade of the tumor <\/b> <span style=\"font-weight: 400;\">(1 to 3), that is, how much cancer cells differ from normal cells and how quickly they can grow. This difference is indicative of the aggressiveness of the disease: grade 1 cancer cells look similar to normal stomach cells, multiply slowly and are less likely to spread; grade 3 cancer cells look very abnormal and multiply rapidly.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The anatomo-pathologist also performs immunohistochemical analysis on the tumor to <\/span><b>obtain information about the genes in its cells, particularly the HER2 gene <\/b> <span style=\"font-weight: 400;\">which regulates the production of a protein involved in cell growth and migration processes. An alteration of this gene (called amplification) causes the overproduction of the HER2 protein and consequently the uncontrolled growth of cancer cells.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Stomach cancers that have elevated values of this protein on the cell surface are termed HER2-positive: these are aggressive tumors, however, which can be treated with a specific therapy, called &#8220;biologic&#8221; or &#8220;molecular-targeted&#8221; therapy.<\/span><\/p>\n<p>Further immunohistochemical analysis involves assessment of expression on tumor cells of PDL1 protein and mismatch repair (MMR) proteins involved in DNA repair functions, the alteration of which confers susceptibility to immunotherapy treatments.<\/p>\n<p><span style=\"font-weight: 400;\">Based on the results of diagnostic tests, the stage of the tumor is also defined, that is, how far the disease has invaded the stomach lining, muscle wall, lymph nodes, or surrounding organs. Stage is calculated by a combination of the categories T (tumor size and invasion of adjacent tissues), N (lymph node involvement) and M (presence of metastasis or spread of the tumor to other organs of the body).<\/span><\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t<\/div>\n\n\t<\/div>\n\n<h2 class=\"wp-block-heading\"><span  id=\"therapies\" class=\"h2_anchor\"><\/span>Therapies<\/h2>\n\n<p>After confirmation of the diagnosis and <strong>staging of the tumor-that<\/strong> is, assessing the extent of the disease to plan the most appropriate treatment-the specialists in the Interdisciplinary Group come together and evaluate a number of factors to plan a <strong>personalized course of treatment<\/strong> for the patient, which varies according to the stage and aggressiveness of the tumor but also according to the patient&#8217;s age and health status.<\/p>\n\t<div id=\"block_6931bb285c98a\" class=\"accordion-block acf-block\">\n\t\t<div class=\"my-4 lg:my-6\">\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Endoscopic resection<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p>If the tumor is located only on the inner mucosa of the stomach, is small in size, and does not appear to have affected the lymph nodes, <strong>it can be removed by endoscopic resection<\/strong>: as is done during gastroscopy, the doctor moves a thin tube-shaped instrument down the patient&#8217;s throat and pushes it down to the stomach after which he removes the tumor.<\/p>\n<p><!-- \/wp:paragraph --><!-- wp:paragraph -->Depending on the histological examination on the excised piece, such resection may be considered sufficient or instead require subsequent surgery.<\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Gastrectomy<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p>This intervention<strong> is the main treatment option<\/strong>, especially in cases of early stage (0-III) cancer.<\/p>\n<p>Surgery may be preceded by chemotherapy to reduce the size or extent of the disease. <strong>Chemotherapy<\/strong> can also be used after surgery, sometimes combined with radiation therapy, to eliminate any remaining cancer cells and reduce the risk of recurrence.<\/p>\n<p><!-- wp:paragraph --><\/p>\n<p>Surgery for stomach cancer&#8211;called <strong>gastrectomy&#8211;can<\/strong> be performed with<strong> different techniques,<\/strong> depending on the extent and aggressiveness of the disease, the age and health status of the patient:<\/p>\n<p><!-- \/wp:paragraph --> <!-- wp:paragraph --><\/p>\n<ul>\n<li><strong>may be partial<\/strong>, and thus involve removal of only part of the stomach, if the tumor has developed in specific locations<\/li>\n<li><strong>can be total<\/strong>, that is, of the entire organ, and possibly of other tissues if invaded by the tumor. <strong>Lymph nodes<\/strong> surrounding the stomach are always removed, however, as they are potential sites of metastasis.<\/li>\n<\/ul>\n<p><!-- \/wp:paragraph --> <!-- wp:paragraph --><\/p>\n<p>The remaining part of the stomach or esophagus is connected directly to the intestines to allow the digestive system to continue functioning.<\/p>\n<p><strong>Minimally invasive surgery<\/strong><\/p>\n<p><span style=\"font-weight: 400;\">At the Candiolo Institute, gastrectomy is performed, when made possible by the extent of the disease and the patient&#8217;s situation, with<\/span><b>minimally invasive surgical techniques <\/b>that reduce side effects and allow rapid patient recovery<span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It is particularly developed the <\/span><b>robotic surgery<\/b><span style=\"font-weight: 400;\">: The surgeon, from a computerized workstation, maneuvers the arms of a robot that terminate miniaturized surgical instruments-with which he makes incisions of a few millimeters-and a 3-D camera. The robot provides the surgeon with high-definition images to facilitate him or her during the procedure, which, because it lacks the physiological tremor of the hand, allows very precise movements to be performed. With this technique, the main advantages are easier removal of lymph nodes and execution of reconstructive sutures with less blood loss and less pain for the patient.  <\/span><\/p>\n<p><span style=\"font-weight: 400;\">After surgery <\/span><b>the patient is taken care of by a nutritionist physician<\/b><span style=\"font-weight: 400;\"> who provides him or her with guidance on proper nutrition and thus prevent weight loss and malnutrition, promote tissue healing and gastrointestinal function.<\/span><\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Chemotherapy<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p><span style=\"font-weight: 400;\">The treatment uses <\/span><b>Drugs aimed at blocking or eliminating cancer cells <\/b> <span style=\"font-weight: 400;\">Taking advantage of their faster reproduction rate than healthy ones. Because it interferes with the replication mechanisms of cells, chemotherapy also damages healthy cells in the body causing major side effects that in most cases disappear once the treatment is over.<\/span><\/p>\n<p><span style=\"font-weight: 400;\"><strong>There are many chemotherapy drugs<\/strong> and they are often used in combination with each other. <\/span><span style=\"font-weight: 400;\">Before therapy begins, the oncologist provides guidance on which drugs to use and what to do to alleviate side effects.<\/span><\/p>\n<p><strong>Method of administration and treatment cycles<\/strong><\/p>\n<ul>\n<li><span style=\"font-weight: 400;\"><strong>The way chemotherapy is administered<\/strong> varies depending on the type of tumor and the drugs: it can be taken <strong>orally<\/strong>, in tablet form, but more often it is administered<strong> intravenously<\/strong>. Intravenous administration is performed on an outpatient basis, and its duration can vary from minutes to hours depending on the drugs used;<\/span><\/li>\n<li>chemotherapy is given <b>in cycles<\/b>: each cycle lasts for a few days and is followed by a few weeks of rest. It aims to explore the mechanisms of response of gastro-oesophageal cancer to treatment with so-called &#8220;targeted&#8221; drugs. After the patient has provided informed consent, unused surgical material after histologic analysis is used to generate molecularly characterized preclinical models (xenopatients) on which to validate drug combinations. The aim is to gain new insights into the interaction between the tumor genome and the efficacy of the therapy patients are subjected to, so as to understand what mechanisms may underlie different drug response and tumor progression.<\/li>\n\n\n\n<li>a clinical trial is underway aimed at <strong>evaluating the use of liquid biopsy<\/strong> to improve response to biologic therapies in metastatic cancer. A liquid biopsy is a test that can detect the genetic-molecular profile of the tumor from a blood sample.
