There are many questions, doubts, and even false myths circulating about immunotherapy. In particular, the forms of immunotherapy most widely used in the clinic today help the immune system <strong>recognize the<br\/>tumor and eliminate it<\/strong>.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"how-does-immunotherapy-affect-the-tumor\" class=\"h2_anchor\"><\/span>How does immunotherapy affect the tumor?<\/h2>\n\n<p>Many tumors are able to &#8220;turn off&#8221; the immune system by blocking lymphocytes through immunological brake signals. The most widely used <strong>immunotherapy<\/strong> drugs, called <strong>immune checkpoint inhibitors<\/strong>, remove these brakes, leaving the lymphocytes free to act against the tumor.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"does-immunotherapy-cure-the-tumor-or-keep-it-under-control\" class=\"h2_anchor\"><\/span>Does immunotherapy &#8220;cure&#8221; the tumor or keep it under control?<\/h2>\n\n<p>Immunotherapy can do <strong>both<\/strong>: in some cases it can lead to complete and lasting responses, in others to disease control, and, unfortunately, in some situations it is not effective. Current <strong>research <\/strong>challenges are aimed at understanding, predicting and modulating the reasons for these differences.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"how-is-it-different-from-chemotherapy\" class=\"h2_anchor\"><\/span>How is it different from chemotherapy?<\/h2>\n\n<p><strong>Chemotherapy directly targets<\/strong> dividing <strong>cells<\/strong>;<strong>immunotherapy acts on the immune system<\/strong>, reactivating its ability to eliminate cancer cells.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"to-what-types-of-cancer-is-it-applicable\" class=\"h2_anchor\"><\/span>To what types of cancer is it applicable?<\/h2>\n\n<p>The first applications, about 15 years ago, were in <strong><a href=\"https:\/\/irccs.com\/en\/patologie\/melanomas\/\" type=\"pathology\" id=\"342\">melanoma<\/a>; <\/strong>later it gradually expanded to different types of cancer. Currently, the possibilities for use are continuously expanding and we find, for example, indications in <a href=\"https:\/\/irccs.com\/en\/patologie\/lung-cancer\/\" type=\"pathology\" id=\"106\"><strong>lung tumors <\/strong><\/a>and in<br\/>Neoplasms of the<strong>urogenital<\/strong> and <strong>gastroenteric<\/strong> systems.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"is-it-suitable-for-all-patients\" class=\"h2_anchor\"><\/span>Is it suitable for all patients?<\/h2>\n\n<p>No, there are specific contexts and situations, related to the characteristics of the tumor or the patient, that physicians must evaluate before placing an indication for immunotherapy.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"why-is-it-proposed-to-some-patients-and-not-to-others\" class=\"h2_anchor\"><\/span>Why is it proposed to some patients and not to others?<\/h2>\n\n<p>It depends on clinical and biological criteria that are evaluated by physicians in accordance with guidelines. Just as an example, some autoimmune diseases present may increase the risk of toxic effects or, again, some cancers may not have characteristics that would<br\/>Be recognized and attacked by the immune system.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"can-immunotherapy-be-done-together-with-chemotherapy-or-radiation-therapy\" class=\"h2_anchor\"><\/span>Can immunotherapy be done together with chemotherapy or radiation therapy?<\/h2>\n\n<p>Yes, in some cases associations are possible and, indeed, may act synergistically.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"how-is-it-administered\" class=\"h2_anchor\"><\/span>How is it administered?<\/h2>\n\n<p><strong>Intravenously<\/strong>, but recently subcutaneous forms are also being evaluated.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"how-often-is-it-done-and-for-how-long\" class=\"h2_anchor\"><\/span>How often is it done, and for how long?<\/h2>\n\n<p><strong>It depends on the drug and the type of tumor<\/strong>. In general, there are biweekly, triweekly or monthly administration schemes.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"is-hospitalization-necessary\" class=\"h2_anchor\"><\/span>Is hospitalization necessary?<\/h2>\n\n<p>No, these are usually therapies administered on <strong>an outpatient<\/strong> basis.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"can-one-continue-to-work-or-carry-out-normal-daily-activities\" class=\"h2_anchor\"><\/span>Can one continue to work or carry out normal daily activities?<\/h2>\n\n<p><strong>Normally yes<\/strong>; of course it depends on the patient&#8217;s clinical condition.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"how-effective-is-immunotherapy\" class=\"h2_anchor\"><\/span>How effective is immunotherapy?<\/h2>\n\n<p>It depends on the tumor and clinical situations. In some contexts, as mentioned above, it has been shown to<strong> cure cancers even in advanced stages<\/strong>; in others it may allow <strong>control of the disease<\/strong> and, unfortunately, <strong>may not be effective in some cases<\/strong>.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"after-how-long-do-you-see-the-first-results\" class=\"h2_anchor\"><\/span>After how long do you see the first results?<\/h2>\n\n<p>Generally <strong>after a few weeks.<\/strong> The timing of the results depends precisely on the fact that its action takes as long as it takes for the immune system to be activated.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"what-happens-if-immunotherapy-does-not-work\" class=\"h2_anchor\"><\/span>What happens if immunotherapy does not work?<\/h2>\n\n<p>It depends a lot on the type of tumor and clinical condition. In general, <strong>alternative strategies are considered<\/strong> whenever possible.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"what-side-effects-can-immunotherapy-give\" class=\"h2_anchor\"><\/span>What side effects can immunotherapy give?<\/h2>\n\n<p>Possible side effects are related to excessive activation of the immune system, which may mistakenly attack healthy organs in addition to cancer cells. These are therefore <strong>effects termed &#8220;autoimmune.<\/strong>&#8221; Examples may include<strong> skin rash, colitis <\/strong>, and some inflammation<br\/>Of the<strong>endocrine<\/strong> glands.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"does-immunotherapy-make-hair-fall-out\" class=\"h2_anchor\"><\/span>Does immunotherapy make hair fall out?<\/h2>\n\n<p>No, and it is usually very well tolerated.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"are-the-side-effects-the-same-for-everyone\" class=\"h2_anchor\"><\/span>Are the side effects the same for everyone?<\/h2>\n\n<p>No, most patients do not manifest any major complaints. Individuals with underlying autoimmune diseases are potentially at greater risk of experiencing side effects. Research is currently trying to understand what factors are associated with different risks and the occurrence of adverse events.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"can-side-effects-appear-even-after-a-long-time\" class=\"h2_anchor\"><\/span>Can side effects appear even after a long time?<\/h2>\n\n<p>Yes, the timing of possible adverse events is <strong>variable<\/strong>. In some cases they may occur early, while in other situations they may be more subtle and appear slowly and over a period of time. In case of new symptoms, it is always recommended to consult with your doctor.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"is-immunotherapy-better-tolerated-than-chemotherapy\" class=\"h2_anchor\"><\/span>Is immunotherapy better tolerated than chemotherapy?<\/h2>\n\n<p><strong>In most cases, yes<\/strong>; of course, this is not generalizable, and it should be noted that today even chemotherapy regimens are generally very well tolerated.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"can-i-travel-during-the-treatment\" class=\"h2_anchor\"><\/span>Can I travel during the treatment?<\/h2>\n\n<p>In general, yes, there are no particular limitations.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"are-there-any-restrictions-on-diet-physical-activity-or-vaccinations\" class=\"h2_anchor\"><\/span>Are there any restrictions on diet, physical activity, or vaccinations?<\/h2>\n\n<p>In general, no; in most cases one can continue to lead a normal life.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"does-immunotherapy-strengthen-the-immune-system\" class=\"h2_anchor\"><\/span>Does immunotherapy &#8220;strengthen&#8221; the immune system?<\/h2>\n\n<p><strong>Not exactly: it modulates it<\/strong>. It removes the brake, that is, the tumor-induced inhibition of the immune system, making the lymphocytes free to attack the neoplastic cells.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"if-i-have-no-side-effects-does-that-mean-it-is-not-working\" class=\"h2_anchor\"><\/span>If I have no side effects, does that mean it is not working?<\/h2>\n\n<p>No, the effectiveness of treatment is monitored <strong>with clinical visits and instrumental examinations <\/strong>(e.g., CT scan, MRI).<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"is-this-an-experimental-therapy\" class=\"h2_anchor\"><\/span>Is this an experimental therapy?<\/h2>\n\n<p>No, immunotherapy understood as immune checkpoint inhibitor drugs (e.g., anti-PD-1 or anti-PD-L1)<strong> is now approved in many cancer settings. <\/strong>New applications or schemes are also part of experimental clinical protocols.<\/p>\n\n<h2 class=\"wp-block-heading\"><span  id=\"is-it-a-very-expensive-therapy\" class=\"h2_anchor\"><\/span>Is it a very expensive therapy?  <\/h2>\n\n<p>The costs of these modulators of the immune system are high.
